What Is GUIDE?

GUIDE is a Medicare program that supports people living with dementia and the caregivers who care for them. Through GUIDE, qualifying seniors receive coordinated dementia care, caregiver support and access to respite services.

How It Works

Getting started with GUIDE is simple:

  1. Makipag-ugnayan sa Amin – Call, text or email to let us know you’re interested in the GUIDE program.
  2. Check Guide Eligibility – To qualify, an individual must be enrolled in Medicare A and B.
  3. Care Conversation – Our team will talk with you about your needs and complete an assessment.
  4. Medicare Approval – Approval typically takes 2–3 weeks.

How to Check Eligibility Using Your Medicare Card

Checking eligibility for the GUIDE program starts with your Medicare card. This card includes the information needed to verify coverage.

What to Look For on Your Card

  • Medicare Number – This is the unique number printed on the front of your card. It is not your Social Security number.
  • Part A and Part B Coverage – GUIDE is available to individuals enrolled in Medicare Part A and Part B.

Your Medicare number is typically shown near your name on the card and may include both letters and numbers.
